Hackensack Meridian Health At Home seeks a Home Care Infusion RN to deliver professional nursing care in patients’ homes. You will assess, implement, and evaluate infusion therapies, ensuring safe, compliant care with an emphasis on patient education and family involvement.

Responsibilities include coordinating with physicians, documenting in the EMR, managing care plans, and supporting payor authorizations. A strong IV infusion background and excellent communication are essential.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• Minimum of one (1) year of RN experience.
• Ability/Experience with inserting IVs.
• Competency in IV access and infusion procedures; vascular access device training may be required.

Responsabilités

• Provide professional nursing care to patients in their homes.
• Assess, plan, implement and evaluate home nursing care needs.
• Coordinate with physicians and interdisciplinary team.
• Document care accurately in the EMR and ensure timely physician orders.
• Maintain patient safety and adherence to standards of care.
• Assist with payor authorizations and care plan updates.
