Job summary

Hackensack Meridian Health At Home seeks a Home Infusion RN to provide professional nursing care in patients' homes. You will manage infusion therapy, follow physician orders, and document care while coordinating with an interdisciplinary team. Expect on-call duties and a patient-centered, flexible home care environment.

Qualifications

• RN with minimum one year of experience in nursing.
• IV insertion experience preferred.
• Competent in infusion therapy and patient education.

Responsibilities

• Provide professional nursing care at patients' homes with infusion therapy.
• Ensure visits follow physician orders and documentation is complete.
• Coordinate with interdisciplinary team and update care plans as needed.
• Document care accurately and communicate plan of care updates.
• Be on-call for after-hours visits and respond promptly to messages.
