Hackensack Meridian Health at Home is seeking a Home Care Infusion RN to deliver professional nursing care in patients' homes. You will assess, plan, implement, and evaluate care, focusing on infusion therapy and patient outcomes in a home setting.

The role requires an RN with at least 1 year of experience, IV insertion skills, and strong communication. A BSN is preferred; NJ RN license is required. Flexible scheduling and comprehensive benefits are offered as part of a total rewards package.

Formación

• Associate's Degree in Nursing or Diploma in Nursing required or equivalent.
• Minimum of 1 year RN experience.
• Ability/Experience with inserting IVs.
• Excellent communication skills and proficiency with documentation.

Responsabilidades

• Provide professional nursing care in patients' homes with assessment, plan, and evaluation.
• Ensure follow-up and documentation align with physician orders and care plans.
• Coordinate with interdisciplinary team and manage authorization processes as needed.
• Document care plans, assessments, and progress in EMR per policy.
