Responsibilities

• Analyze unit level quality data and participate in improvement efforts.
• Collect, assess, and document patient healthcare data.
• Direct the patient/family and interdisciplinary team to identify a plan of care.
• Prioritize care based on collected data and document assessments.
• Direct the coordination of patient care in a safe and timely manner.
• Educate the patient/family regarding health status and plan of care.
• Collaborate with the interdisciplinary team to modify expected outcomes.
• Evaluate the patient’s progress towards expected outcomes.
• Recognize changes in patient condition and modify interventions as needed.
• Serve as a support, resource, and preceptor for nursing staff and students.
