Job summary

Amerita, a BrightSpring Health Services affiliate, seeks a Pharmacist for our High Point branch to provide pharmaceutical care for home infusion patients. You will collaborate with physicians and nurses to optimize drug therapy and ensure compliance with regulations.

You will evaluate drug therapies, review orders, and document clinical activities while supporting referrals and patient care in the home setting. Professional development discussions are offered.

Qualifications

• Pharmacist with doctor of pharmacy or bachelor of science in pharmacy.
• Active pharmacist license in applicable state(s).
• Experience in home infusion or acute-care setting preferred.

Responsibilities

• Evaluate and monitor drug therapy for home care patients.
• Adhere to state/federal pharmacy regulations and organization policies.
• Coordinate referrals and admissions with nurses and distribution.
• Review and verify new patient orders and document appropriately.
• Provide drug information and counseling to patients and healthcare professionals.
• Maintain documentation of clinical activities and communications.
