Job summary

Soleo Health in Mobile, AL is looking for a Full-time Infusion Patient Care Coordinator to support our Specialty Infusion Pharmacy on-site. The role focuses on data entry, scheduling coordination for the nursing team, and ensuring a smooth patient experience.

Responsibilities include coordinating referrals, obtaining benefits information, calling patients and prescribers, and maintaining accurate schedules.

Qualifications

• Prior healthcare scheduling experience is required.
• Home infusion experience preferred.
• Ability to work on-site and interact directly with patients in a clinical setting.
• Strong communication skills.
• Ability to prioritize and multitask.
• Basic computer skills including Microsoft Excel, Word, Outlook.
• High school diploma or equivalent required.

Responsibilities

• Data entry of clinical information into the management system for billing.
• Coordinate scheduling activities for the nursing department and ensure accuracy.
• Contact referral sources, patients, or prescribers to obtain benefits information or prior approvals.
• Place outbound calls to patients or doctors to notify them of care status.
• Coordinate and plan scheduling for nursing visits across geography.
• Provide exceptional customer service to internal and external stakeholders.
