Soleo Health is seeking a Full-time Infusion Care Coordinator (Patient Ambassador) to support our specialty infusion pharmacy in Pewaukee, WI. This on-site role focuses on data entry, scheduling coordination, and patient communications to ensure timely care and positive patient experiences.

Responsibilities include managing referrals, coordinating with nursing teams, and maintaining accurate scheduling. Strong communication and basic computer skills are essential to provide exceptional customer

Qualificações

• Prior healthcare scheduling experience required.
• Home infusion experience preferred.
• Ability to work on-site and interact with patients in a clinical setting.
• Strong communication skills.
• Ability to prioritize and multitask.
• Basic computer skills including Excel, Word, Outlook.
• High school diploma or equivalent required.

Responsabilidades

• Enter clinical data into the management system to support billing and scheduling.
• Coordinate nursing department scheduling and related tasks.
• Call referral sources, patients, and prescribers to obtain needed information.
• Place outbound calls to notify patients of care status.
• Collaborate with prescribers to facilitate payor denials and appeals.
• Act as main contact for referral activity with internal and external partners.
• Provide excellent customer service and resolve requests promptly and accurately.
• Notify patients about financial responsibility and insurance coverage for services.
• Coordinate and plan scheduling for the nursing team and ensure efficiency.
• Support AIS follow-ups and ensure seamless patient experiences.
