Job summary

Fred Hutchinson Cancer Center is seeking a Per-Diem Pharmacist for the Peninsula Clinic in the Seattle area. You will collaborate with providers to optimize therapies, perform MTM reviews, and manage patient care with the specialty pharmacy team.

The role requires WA Pharmacist License and a PharmD or equivalent, with at least two years of practice; a per-diem, variable schedule applies Monday through Friday.

Qualifications

• Two years of pharmacy practice experience (months in ASHP-accredited residency may substitute)
• State of Washington Pharmacist License required
• Graduation from an accredited School of Pharmacy required

Responsibilities

• Collaborate with providers to tailor drug therapies for safe, effective care.
• Perform comprehensive medication therapy reviews and transitions of care.
• Monitor and adjust regimens; order and interpret labs as needed.
