Job summary

Fred Hutchinson Cancer Center in Seattle area is seeking a clinical pharmacist for a 0.5 FTE, regular part-time schedule at our Issaquah location. The role includes direct patient care, dispensing, research and education, and collaboration with the health care team to optimize therapy.

Required Washington pharmacist license and pharmacy degree with at least 2 years of practice; PharmD preferred. Pre-employment drug screening applies; comprehensive benefits are provided.

Qualifications

• Graduation from an accredited School of Pharmacy.
• State of Washington Pharmacist License.
• Two years of experience in pharmacy practice.

Responsibilities

• Collaborate with providers to tailor drug therapies for safe, effective patient care.
• Perform comprehensive medication therapy reviews and ensure safe transitions of care.
• Monitor and modify drug regimens, including lab testing when needed.
• Provide refill authorization and educate patients on therapy.
• Support anticoagulation, infectious disease therapy, and other specialties when needed.
• Maintain communication across pharmacists, physicians and nurses.
