Job summary

Fred Hutchinson Cancer Center in Issaquah, WA is seeking a licensed pharmacist to join our clinical pharmacy team on a .5 FTE schedule, Monday through Friday, at our Issaquah location.

You will collaborate with providers, nursing and research staff to optimize patient care, perform medication therapy review, monitoring, and provide drug information while supporting education and research initiatives across departments.

Qualifications

• Graduation from an accredited School of Pharmacy
• State of Washington Pharmacist License
• Two years of experience in pharmacy practice (ASHP residency may substitute)

Responsibilities

• Collaborate with providers to individualize drug therapies
• Perform medication therapy reviews and ensure safe care transitions
• Monitor and modify drug regimens as needed; order/interpret labs
• Provide refill authorizations and act as drug information resource
• Ensure access to specialty medications and support adherence
