Job summary

Fred Hutchinson Cancer Center in the Seattle area is seeking a pharmacist for a per-diem role at the Peninsula Clinic. The pharmacist will work a variable schedule Monday through Friday, performing clinical pharmacy services, direct patient care, dispensing and providing pharmacotherapeutic recommendations within a collaborative health care team.

Qualifications

• Pharmacotherapy and clinical pharmacy services in a hospital setting.
• Licensing and credentialing as a Washington pharmacist are required.
• Experience with residency training program may substitute for experience.

Responsibilities

• Collaborate with providers to individualize drug therapies for safe, effective care.
• Perform medication therapy review and ensure safe transitions of care.
• Monitor and modify drug regimens with lab testing when indicated.
• Provide refill authorization with adherence and safety assessment.
• Serve as a drug information resource to providers and staff.
