Job summary

Option Care Health is seeking an Infusion Nurse II to deliver intermediate direct patient care in the home or an infusion suite, following a physician's plan of care. This role requires an active RN license in the state of practice and CPR certification, with a minimum of one year of nursing experience.

Weekend shifts are available. You will document care in real time, coordinate with the clinical team, mentor less experienced nurses, and ensure compliance with HIPAA, patient safety, and

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 years of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments and educates patients and families.
• Assesses patient needs and status at each visit and documents changes for physician orders.
• Provides training and mentorship to Infusion Nurse I’s.
• Provides effective teaching and supports patient and family independence with prescribed therapy.
• Communicates with the Clinical team, medical providers, patients, and families to facilitate continuity of care and submits real-time documentation.
• Coordinates discharge planning and prepares discharge summaries with patient instructions.
• Demonstrates compliance with agency policies, procedures, and regulatory standards.
• Maximizes work efficiency using computers to validate plan of treatment orders and document care.
• Responds to patient requests and initiates emergency procedures when necessary.
