Job summary

Option Care Health, Inc. is seeking an Infusion Nurse II to coordinate and deliver intermediate direct patient care in the home or infusion suite under a physician’s plan. Licensed RN with at least 1 year nursing experience is required.

You will educate patients and families, document care in real-time, collaborate with the clinical team, and support discharge planning while adhering to HIPAA and safety standards. Strong communication and computer skills are essential.

Qualifications

• Active RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates and implements intermediate nursing plan of care treatments.
• Assesses patient needs and status during visits and documents findings.
• Provides training and mentorship to Infusion Nurse I’s as needed.
• Educates patients and families for independence with prescribed therapy.
• Communicates with clinical team, physicians, and families to coordinate care.
• Coordinates discharge planning and prepares discharge summaries.
• Ensures compliance with policies, regulations, and accreditation standards.
• Utilizes technology to validate orders and document care.
• Performs other duties as directed by supervisor.
