Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate home infusion care under a physician’s plan of care. The role requires licensed RN status and strong patient education skills.

You will work with a multidisciplinary team to ensure safe, effective care in patients’ homes or alternate infusion suites, with emphasis on documentation, safety, and timely communication.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments.
• Assesses patient needs and physical status at each skilled visit.
• Provides training and mentorship to Infusion Nurse I's as needed.
• Provides effective teaching for patient and family to achieve independence with prescribed therapy.
• Communicates with Clinical team, medical providers, patients, and families to facilitate continuity of care.
• Coordinates discharge planning and prepares discharge summaries.
