Job summary

Soleo Health is seeking an Infusion Nurse to provide primary patient care in home or alternate sites, including assessments, care planning, and education for infusion therapy patients. You will collaborate with pharmacists, prescribers, dietitians, and care partners, ensuring safe home infusion administration and timely documentation.

Responsibilities include evaluating patient environments, coordinating care, and participating in on-call rotations.

Qualifications

• Current Registered Nurse (RN) license in good standing with the State of Kansas and Missouri Board of Nursing.
• BSN preferred from an accredited program.
• Minimum of 3 years of clinical nursing experience, infusion therapy preferred.
• Experience with peripheral IV (PIV) insertion and vascular access (ports/PACs).
• Experience administering infusion therapies (biologics, IV hydration) preferred.

Responsibilities

• Provide direct care for assigned patients including initiation and maintenance of communication with care team members.
• Educate patient/care partner to safely administer home infusion therapy.
• Assess home environment and ensure safety for prescribed care.
• Develop and evaluate plan of care with the team.
• Manage schedule and coordinate patient care via approved platform.
• Participate in nurse on-call rotation for continuous support.
• Document all encounters and maintain compliance with policy and regulations.
• Maintain knowledge of Soleo therapies, protocols, and clinical standards.
