Job summary

Soleo Health is seeking an Infusion Nurse in Arizona to deliver primary patient care for home infusion therapy. You will perform assessments, care planning, and education, coordinating with a multidisciplinary team to ensure safe, effective treatment in patients’ homes.

Responsibilities include on-call rotation, documenting care, and maintaining knowledge of Soleo therapies and clinical standards. A valid AZ RN license and 3+ years infusion experience are preferred.

Qualifications

• Current RN license by the state board, in good standing.
• BSN from an accredited institution preferred.
• Advanced certification in infusion/nursing specialties preferred.
• Minimum 3 years nursing experience with infusion therapy background.
• Experience in ICU and/or Cardiac Care preferred.
• Proficient with Microsoft Outlook and MS Office.

Responsibilities

• Provide direct care for all assigned patients including initiation and maintenance of communications with care team members.
• Evaluate and educate patient/care partner to safely administer home infusion therapy.
• Assess the home environment and identify safety interventions for prescribed care.
• Develop with the team the plan of care and evaluate response to therapy.
• Manage schedule via company approved platform and communicate scheduling changes.
• Coordinate patient care via approved company platform.
