Job summary

Soleo Health is seeking a per diem Infusion Nurse to deliver primary patient care for home infusion therapy, including assessments, care planning, and education. You will collaborate with pharmacists, prescribers, and care partners to ensure safe, effective treatment in patients' homes.

The role requires an active RN license in Illinois, BSN preferred, and experience in infusion therapy or ICU/Cardiac care.

Qualifications

• Current RN license in good standing in the applicable state.
• BSN preferred.
• Advanced certifications in specialty areas (e.g., CRNI, ONS, IgRN) preferred but not required.

Responsibilities

• Provide direct patient care including initiation and maintenance of communications with care team members.
• Educate patient/care partner to safely administer home infusion therapy.
• Evaluate the home environment and identify safety interventions.
• Develop and evaluate plan of care with the team.
• Manage scheduling and coordinate patient care via approved platform.
• Participate in nurse on-call rotation.
• Document encounters and care activities per policy.
• Maintain knowledge of Soleo therapies and clinical standards.
• Submit plans of treatment and required documents timely.
