Job summary

Soleo Health in Roswell, GA is seeking a full-time Infusion Nurse to provide direct patient care for infusion therapy in home or alternate sites. You will perform assessments, care planning, and education for assigned patients in accordance with care matrices and best practices.

The role requires collaboration with pharmacists, prescribers, and care partners, maintaining documentation, and participating in on-call rotation.

Qualifications

• RN license in the state; active and in good standing.
• BSN degree preferred; clinical infusion experience preferred.
• Advanced infusion certifications (CRNI/ONS/IgRN) preferred but not required.
• On-call rotation including nights, weekends, or holidays as needed.
• Proficient with scheduling and coordination in a healthcare setting.

Responsibilities

• Provide direct care for assigned patients including initiation and communication with care team.
• Educate patient/care partner to safely administer home infusion therapy.
• Assess home environment and intervene to ensure safe care delivery.
• Develop and evaluate the care plan with team members.
• Manage schedule via company platform and communicate changes promptly.
• Coordinate patient care using approved platforms.
• Participate in nurse on-call rotation for continuous support.
• Document all patient encounters and care activities per policy.
• Maintain knowledge of Soleo therapies, protocols, and standards.
• Submit POTs and required clinical documents per policy and state guidelines.
• Ensure all patient documents are completed and returned timely to branch.
