Job summary

Option Care Health, Inc. seeks an Infusion Nurse Educator to educate patients, caregivers, and facility staff about infusion services and to ensure smooth transitions to at-home care.

You will assess patient viability for the care model and collaborate with the sales team to support clinical transitions. You will document education in OCH systems, coordinate discharge planning, and provide mentorship to other nurses while adhering to HIPAA and regulatory guidelines.

Qualifications

• Active and unrestricted RN license with CPR certification.
• BSN preferred; multi-state licensure readiness may be required.
• Strong patient education and communication skills.
• Experience with discharge planning and home infusion care.
• Proficient with basic MS Office tools for documentation.

Responsibilities

• Educate patients, caregivers, and staff on OCH services and home infusion.
• Assess patient eligibility to join the OCH care model.
• Coordinate with the sales team to support clinical transitions.
• Document care plans and ensure timely discharge, education, and billing.
• Provide supervision and mentoring to nurses as assigned.
