Job summary

Option Care Health is seeking an Infusion Nurse Educator to deliver clinical education and training for patients transitioning to our care model. The role also provides intermediate direct patient care at home or in an infusion suite, ensuring safety and continuity under a physician's plan of care.

You will partner with the sales team to facilitate transitions, assess patient viability, and educate staff and caregivers to optimize patient outcomes while safeguarding confidentiality and complying

Qualifications

• Licensed Registered Nurse (RN).
• Experience in infusion nursing and patient education.
• Ability to document and coordinate care across teams.

Responsibilities

• Provide intermediate direct patient care in home or alternate infusion setting.
• Educate patients, caregivers, and facility staff about OCH services and products.
• Coordinate with sales team to transition patients to OCH care model.
• Maintain confidentiality and follow legal guidelines for patient information.
• Document education using OCH applications and ensure timely discharge planning.
