Option Care Health is seeking an Infusion Nurse Educator to deliver clinical education and direct patient care in home or alternate infusion settings. You will educate patients, caregivers, and staff on the care model and ensure safe, compliant transitions under physician plans of care.

Responsibilities include patient assessments, collaboration with the sales team for education needs, and meticulous documentation to support billing and continuity of care.

Qualifications

• Active and unrestricted RN license required.
• Minimum of 2 years nursing experience.
• Current CPR certification required.
• High School diploma or GED required.

Responsibilities

• Educate patients, caregivers and facility staff on OCH services and infusion care at home or alternate sites.
• Assess patient viability and coordinate with sales for smoother transitions to OCH.
• Document education and ensure timely discharge planning and continuity of care.
• Maintain HIPAA-compliant confidentiality and liaise with clinical teams.
