Option Care Health seeks a Clinical Transition Specialist to educate patients and healthcare staff on home infusion services and to ensure proper patient placement at alternative sites. You will build relationships with referrals, work with Account Managers to grow referrals, and meet sales goals while coordinating with discharge planners and internal teams.

The role emphasizes hands-on patient education, timely discharge, and collaboration across hospital and home-based care settings.

Qualifications

• Licensed RN or Pharmacist in state of practice.
• 2+ years of healthcare experience.
• Experience providing customer service and meeting quality standards.
• Basic PC skills: Word, Excel, PowerPoint.

Responsibilities

• Proactively maintains and grows relationships with referrals sources to increase sales and patient starts.
• Serves as key point of contact and representative of Option Care to referral sources.
• Interacts with area service providers to sell Option Care services for discharging patients.
• Conducts assessment of patients to ensure suitability for alternate site care.
• Educates patients, families and facility staff about services and products.
• Provides in-person education to patients and caregivers for timely discharge.
• Communicates with agency staff, medical team, patients and family during discharge.
