Job summary

Option Care Health seeks a Clinical Transition Specialist to educate patients, families, and facility staff about home infusion services. You will ensure proper placement of patients in home health settings and coordinate with discharge planners to support seamless transitions.

This role focuses on building strong referral relationships, partnering with Account Managers to grow referrals, and meeting sales goals while delivering compassionate, high‑quality care across the United States.

Qualifications

• Experience establishing relationships with diverse stakeholders at all levels.
• Proven ability to educate customers and healthcare staff about services.
• Strong communication and presentation skills for sales contexts.
• Familiarity with ACHC/URAC standards and healthcare regulations.

Responsibilities

• Grow referrals and patient starts by engaging hospital and facility staff.
• Educate patients, families and staff on home infusion services and products.
• Coordinate with intake, discharge planners and account managers to ensure smooth transitions.
