Job summary

Option Care Health, Inc. is seeking a Clinical Transition Specialist to educate patients and care teams about home infusion services and to place patients in appropriate alternative-site care.

You will work with hospital staff, discharge planners and referral sources to ensure a smooth transition and timely discharge. The ideal candidate holds an RN or Pharmacist license in the practice state with at least two years in healthcare and has strong relationship-building and sales skills to meet

Qualifications

• LicensedRN or Pharmacist in-state practice with 2+ years healthcare experience.
• Experience building relationships with organizations at multiple levels.
• Knowledge of ACHC/URAC standards and patient education basics.

Responsibilities

• Grow referrals and patient starts; educate patients and families.
• Coordinate with hospital staff, intake, case managers, and discharge planners.
• Provide in-person education to patients and caregivers for timely discharge and therapy independence.
• Collaborate with Account Managers to expand referral rates and sales goals.
