Job summary

Option Care Health in Maryland is seeking a Clinical Transition Specialist, an experienced sales-oriented professional who educates patients, families and facility staff about home infusion services and products for discharge planning.

You will build relationships with referral sources, coordinate with hospital partners and Account Managers to grow referrals and meet sales goals while ensuring timely patient transitions to home or alternate sites.

Qualifications

• RN or Pharmacist licensed in practice state with at least 2 years in healthcare.
• Experience establishing and maintaining relationships with individuals at all levels of the organization and with vendors.
• Knowledge of ACHC, URAC standards and healthcare compliance.
• Customer service excellence and ability to engage internal/external customers.
• Proficiency with MS Office and ability to plan and present information.

Responsibilities

• Proactively maintains and grows relationships with referrals sources to increase sales and patient starts.
• Serves as key contact to provide education and service to referral sources.
• Interacts with hospitals and providers to promote Option Care services.
• Assesss patients to determine candidacy for home/alternate site care.
• Educates patients and families about services for timely discharge and independence.
• Coordinate with Account Managers to grow referral rates and hit sales goals.
• Ensure confidentiality and proper handling of medical information.
• Travel to care facilities and partner sites as needed.
