Job summary

Option Care Health, Inc. is seeking a Clinical Transition Specialist to drive sales of home infusion services to discharge patients and educate patients, families, and facility staff on how services are facilitated at an alternative site.

The role builds and maintains referral relationships, coordinates with hospital staff and internal partners, and collaborates with Account Managers to grow referrals and achieve sales goals while ensuring a smooth transition to home health care.

Qualifications

• Must be licensed RN or Pharmacist in state of practice.
• Minimum 2 years in healthcare industry.
• Experience building relationships with stakeholders at all levels.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Serves as key point of contact and representative to provide education and service to referral sources.
• Interacts with area service providers daily to sell Option Care services for discharge planning.
• Conducts patient assessments to ensure candidacy for alternative site care.
• Educates patients and families about services and products offered.
• Coordinates pre-admission information and secures related signoffs.
• Collaborates with Account Manager and Regional Sales Director on area growth plans.
• Reviews medical records for history and insurance payor information.
• Initiates care transition coordination with referral sources and internal partners.
• Facilitates transfer of orders via phone, fax, and e-prescribing.
• Maintains confidentiality and adheres to legal guidelines.
