Job summary

Option Care Health seeks a Clinical Transition Specialist to educate patients and families about home infusion services and ensure smooth discharge to an alternative site. You will work with discharge planners, case managers and internal teams to place patients appropriately and achieve strong referral growth.

The role requires an RN or Pharmacist licensed in the practice state with at least two years in healthcare, plus excellent relationship and communication skills.

Qualifications

• Licensed RN or Pharmacist in the state of practice with 2+ years in healthcare.
• Experience establishing relationships with clinicians, discharge planners and case managers.

Responsibilities

• Proactively maintains and grows relationships with referrals sources to increase sales and patient starts.
• Educates patients and families about home infusion services at discharge.
• Coordinate with internal partners to ensure timely patient transitions to home or ATS.
• Assist with gathering preadmission information and securing required signoffs.
