Pursue your passion for caring with Duke HomeCare & Hospice, which offers hospice, home health, and infusion services across the region, as well as serves as the home forthe Duke Caregiver Support Program. Team members work closely with a patient's physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham, NC.
The Care Coordinator is responsible for the assessment and coordination of home health, hospice, and infusion services for the hospitalized patient, clinic/outpatient and/or patient in an alternative setting (i.e. extended care facilities) by acquiring admission information and assessing for the appropriateness of home care services. The primary focus of this role is coordinating Infusion referrals for adult and pediatric patients within the 3 Duke Hospitals as well as teaching the patient/caregiver the information related to the infusion device used in the home. The RN Care Coordinator functions as an integral part of an interdisciplinary team, Responsible for providing community education, responding to customer requests and concerns, and interfacing with referral sources, organizations, government agencies and third party payors.
Rotating to Duke University, Duke Regional, and Duke Raleigh Hospitals
M-F (5) 8 hour days-Rotating call schedule every 4-6 weeks to cover weekend staff on PTO. Some Holiday Coverage
BSN required. Current RN licensure in the state of North Carolina,or compact licensure in participating stateis required.
BLS required, but if not current may be renewed during orientation.
Three years of nursing experience are required.
