Job summary

Duke HomeCare & Hospice in Durham seeks a dedicated Care Coordinator to oversee the coordination of home health, hospice, and infusion services for patients at Duke Raleigh Hospital. You will assess inpatient home care needs, educate patients and families, and help ensure safe transitions from hospital to home.

Working with interdisciplinary teams and community partners, you’ll document activities in Maestro and CareTend, support discharge planning, and coordinate services to prevent insurance

Qualifications

• RN licensure in NC or compact state required.
• BSN preferred.
• BLS certification required or can be renewed during orientation.

Responsibilities

• Assess inpatient home care needs and initiate appropriate referrals.
• Educate patients and families on home care, hospice, and infusion options.
• Coordinate with hospital Case Manager to request additional services.
