Soleo Health is hiring a Full-time Infusion Care Coordinator (Patient Ambassador) in Woodridge, IL. You will support the Specialty Infusion Pharmacy by entering clinical data for billing, coordinating nursing schedules, and managing referrals with patient-focused service.

Responsibilities include coordinating care steps post AIS visits, collecting insurance information, and communicating with patients and providers to ensure timely dosing and a seamless patient experience.

Qualifications

• Prior healthcare scheduling experience required.
• Home infusion experience preferred.
• Ability to work on-site and interact directly with patients in a clinical setting.

Responsibilities

• Enter clinical information into the clinical management system for billing.
• Coordinate scheduling activities for the nursing department and related tasks.
• Receive referrals and collect insurance information for verification of benefits.
• Place outbound calls to patients or providers to update care status.
• Serve as single point of contact for referral activity across departments.
