Soleo Health in Centennial, CO is seeking a Full-time Infusion Care Coordinator (Patient Ambassador) to support our Specialty Infusion Pharmacy. You will manage data entry, scheduling, and patient coordination to ensure accurate care delivery and timely medication dispensing.

The role requires on-site collaboration with nursing staff, strong communication, and the ability to multi-task in a fast-paced clinical setting.

Qualifications

• High school diploma or equivalent required.
• LPN/LVN license preferred.
• Prior healthcare scheduling experience required.
• Home infusion experience required.
• Proficient with Microsoft Excel, Word, Outlook.
• Ability to interact with patients on-site.
• Strong communication and multitasking skills.

Responsibilities

• Receive medication referrals and collect insurance information.
• Data entry into clinical management system for billing.
• Coordinate scheduling activities for the nursing department.
• Contact referral sources, patients, or prescribers to obtain information.
• Place outbound calls to notify care status.
• Collaborate with prescribers to support payor denials.
• Serve as single point of contact for referral activity with internal customers, manufacturers/HUBs, patients, prescribers, and referral sources.
• Provide exceptional customer service to internal and external customers.
• Notify patients about financial responsibility and coverage.
• Meet with patients post AIS visits to schedule next appointment.
• Ensure accurate scheduling to support timely medication dispensing.
• Promote, foster, and maintain relationships across departments and company lines.
• Support client satisfaction and account retention.
