Soleo Health is seeking a Full-time Patient Infusion Care Coordinator (Patient Ambassador) to support our Specialty Infusion Pharmacy in Dublin, OH. The role focuses on data entry for billing, scheduling for the nursing department, and coordinating patient care with multiple teams to ensure a seamless patient experience.

Responsibilities include collecting eligibility information, coordinating referrals, and maintaining accurate schedules while delivering excellent customer service.

Qualifications

• High school diploma or equivalent required.
• Prior healthcare scheduling experience required.
• Home infusion experience required.
• Strong communication skills.
• Ability to prioritize and multitask.

Responsibilities

• Receive medication referrals and collect insurance information.
• Proficient data entry and generating daily office communications.
• Contact referral sources, patients, or prescribers to obtain information for verification of benefits or prior approvals.
• Place outbound calls to patients or doctor’s offices to notify care status.
• Collaborate with prescribers to facilitate payor denial appeals.
• Act as single point of contact for all referral activity with internal and external partners.
• Provide exceptional customer service to external and internal customers, resolving requests timely.
