Job summary

Advocate Health in Charlotte, NC, seeks a Program Coordinator Infectious Disease to support patient navigation and care coordination. You will collaborate with medical teams, manage referrals, assist with documentation, and ensure compliant records while communicating with pharmacies and agencies.

Qualifications

• Requires bachelor's degree in social work, psychology or related field.
• One year of counseling or social work experience, or three years without a degree.
• Healthcare customer service experience and sensitivity to trauma.
• Experience with EHR systems to identify risk and readmission rates.
• Proficiency with Excel features like PivotTables, VLOOKUP and conditional formatting.
• Comply with health and safety standards.

Responsibilities

• Coordinate and evaluate services for client needs to empower access to resources.
• Send referrals and orders from physicians and agencies; act as a resource to physicians and administration.
• Meet with clinic patients to assess needs and assist with paperwork or applications.
• Maintain clinic databases and ensure compliance with regulations and standards.
• Communicate with pharmacies and state/county agencies to troubleshoot medication access or billing.
• Work M-F from 8 am – 5 pm.
