Job summary

Arcare is seeking a Patient Care Coordinator-Infections Disease in Little Rock, AR to ensure comprehensive care coordination for HIV/AIDS clients. You will partner with patients, families, healthcare providers, and stakeholders to manage care plans, facilitate communication, and connect patients with essential resources and referrals.

The role emphasizes coordination, communication, and advocacy, with a schedule Monday–Friday, 8:00 AM–5:00 PM, full-time employment at Arcare.

Qualifications

Bachelor's Degree is required.
2-4 years of related experience required.
Strong understanding of healthcare systems, insurance, and community resources.

Responsibilities

Oversee the coordination of patients care while serving as a primary contact for patient and their families.
Assist with scheduling, transportation, and connecting patients to community resources and support programs.
Track patient progress, while identifying and resolving care coordination gaps to enhance patient outcomes.
Participate in ongoing training related to care coordination and quality.
