Job summary

The Advocate Health program coordinator role in Charlotte, NC, focuses on coordinating the continuum of care for patients and families. You will liaise with clinical teams, manage referrals, collect and verify documentation, and maintain accurate patient records to meet regulatory and payer requirements.

You will also support communication with pharmacies and external agencies, ensure timely data entry, and comply with health and safety standards while working a standard M-F 8am-5pm schedule.

Qualifications

• Bachelor's degree in Social Work, Psychology or related field or foreign equivalent.
• One year of counseling or social work experience, or social work experience, while complying with applicable health and safety standards.
• In lieu of a degree, three years of counseling or social work experience may be accepted.

Responsibilities

• Function as a liaison for patient/family in navigating the continuum of care.
• Send and receive referrals and orders from physicians and other agencies.
• Meet with clinic patients to assess needs, collect documents, or assist with paperwork or applications.
• Complete accurate and appropriate documentation, logs, and/or patient forms to ensure compliance with regulations and standards of care.
• Maintain statistics and update clinic and country databases with correct patient information for compliance and audit purposes.
• Communicate with pharmacies and state and county agencies to troubleshoot issues of patient medication access or billing.
