Job summary

Tennessee Community Services Agency in Memphis, TN seeks a Case Manager to provide case management services for individuals with developmental and intellectual disabilities enrolled in the Katie Beckett Part B Program.

You will conduct meetings, develop and write ISPs, submit plans to DDA, monitor services monthly, maintain documentation, and coordinate with families and providers.

Qualifications

  • Bachelor's degree required; preferred fields include Social Work, Psychology, Sociology, or a related field.
  • One to two years of related work experience preferred.
  • ISC experience and/or certification is preferred.
  • Knowledge or experience with DDA and/or the Katie Beckett Program is a plus.
  • Strong organizational and time-management skills with the ability to work independently and meet deadlines.
  • Excellent written and verbal communication and documentation skills.

  • Must have reliable transportation and current automobile insurance.
  • Must successfully complete all required background checks and authorize the release of applicable investigative and criminal records obtained through the Federal Bureau of Investigation (FBI) and Tennessee Bureau of Investigation (TBI) to Tennessee Community Services Agency.
Travel Requirements
  • Travel is required for this position. The position serves individuals across a large geographical area. However, the employee will not be required to travel to every location within the assigned service area each month. Travel will vary based on caseload, scheduled meetings, monitoring requirements, and the needs of the individuals served.
