Job summary

FeldCare Connects in Sun Valley, CA seeks a dedicated Physical Therapist to deliver home health services as an independent contractor. You will assess patients, develop individualized treatment plans, and work with adults of all ages to improve movement, reduce pain, restore function, and prevent disability.

You’ll enjoy a flexible schedule, the autonomy of being your own boss, and support with scheduling and care coordination.

Qualifications

• Ability and enthusiasm in working with all ages
• Physical Therapist license and registration by the state
• Curriculum approved by The American Physical Therapy Association, The Council on Medical Education and Hospitals of the American Medical Association, or The Council on Medical Education of the American Medical Association
• Bilingual a plus!

Responsibilities

• Assess patient care needs and determine a treatment plan
• Execute interventions to improve movement and reduce pain
• Monitor patient progress and adjust plans as needed
