FeldCare Connects is seeking a self-motivated Physical Therapist to join our Home Health team serving the Sun Valley area. This Independent Contractor role focuses on delivering premier care to adults of all ages, promoting movement, reducing pain, restoring function, and preventing disability.

You will assess patient needs, determine a treatment plan, implement interventions, monitor progress, and maintain accurate records.

Formación

• Licensed Physical Therapist with state registration.
• Completion of a PT curriculum approved by relevant bodies (APTA/AMA).
• Ability to work with adults of all ages; bilingual a plus.

Responsabilidades

• Assess and evaluate patient care needs to promote movement, reduce pain, restore function, and prevent disability.
• Develop and implement a treatment plan, execute interventions, and monitor progress.
• Document care and adjust plans as needed; communicate with supervising clinician as required.
