Job summary

FeldCare Connects in Yucca Valley, CA is seeking a self-motivated Physical Therapist to deliver premier home health care to adults of all ages as an independent contractor.

You will assess and evaluate patient needs, determine a treatment plan, perform interventions to improve mobility and reduce pain, and monitor progress while keeping accurate records and communicating clearly with patients and care teams.

Qualifications

• Licensed Physical Therapist registered in the state.
• Completion of an APA/ CMAA or related PT curriculum approved by established bodies.
• Experience with patients across all ages is preferred; bilingual a plus.

Responsibilities

• Assess and evaluate patient care needs to promote mobility, reduce pain, restore function and prevent disability.
• Develop and implement individualized treatment plans and execute interventions to improve patient well-being.
• Monitor patient progress and adjust plans as needed while maintaining accurate records and clear communication.
