Job summary

Optum is seeking a Nurse Practitioner or Physician Assistant as the Senior Advanced Practice Clinician for Community Care, serving medically complex patients in skilled nursing facilities across Washington and Multnomah Counties, OR. You will be the primary clinical decision-maker, with an interdisciplinary team and no RVU quotas.

Qualifications

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
• Active and unrestricted NP license in the state of residence, or ability to obtain by start date.
• Board certification through AANP or ANCC in an approved NP specialty.
• Graduate of an accredited Physician Assistant program.
• Active and unrestricted PA license in the state of residence, or ability to obtain by start date.
• NCCPA certification.
• Current DEA licensure/prescriptive authority or ability to obtain per state regulations.

Responsibilities

• Conduct comprehensive patient assessments.
• Manage acute and chronic conditions, diagnostics, and interventions.
• Develop and oversee complex treatment plans.
• Manage patients during short-term sub-acute rehabilitation stays.
• Lead advanced illness, goals-of-care, and advance care planning discussions.
• Collaborate with PCPs, caregivers, facility staff, and interdisciplinary partners.
• Provide patient and caregiver education.
• Ensure accurate and complete clinical documentation.
