Job summary

Optum is seeking an Advanced Practice Clinician (NP/PA) for Senior Community Care in Washington and Multnomah Counties, OR. This field-based, full-time role lets you practice at the top of your license, make independent clinical decisions, and manage your own patient panel with strong interdisciplinary support.

You will conduct comprehensive assessments, manage acute and chronic conditions, and develop complex treatment plans for medically complex patients in skilled nursing facilities.

Qualifications

• Graduate degree required (MSN/NP or PA program).
• Active and unrestricted license in state of residence or ability to obtain by start date.
• Board certification through AANP/ANCC (NP) or NCCPA (PA).

Responsibilities

• Conduct comprehensive patient assessments.
• Manage acute and chronic conditions, diagnostics, and interventions.
• Develop and oversee complex treatment plans.
• Manage patients during short-term sub-acute rehabilitation stays.
• Lead advanced illness, goals-of-care, and advance care planning discussions.
• Collaborate with PCPs, caregivers, facility staff, and interdisciplinary partners.
• Provide patient and caregiver education.
• Ensure accurate and complete clinical documentation.
