Job summary

Care Connectors in Modesto, CA is seeking Nurse Practitioners for in-home Annual Wellness Visits for Medicare members. You’ll work on a flexible schedule, travel to patients' homes, and focus on preventive care without prescribing or treatment plans.

Roles are contract-based with 1099 options and full- or part-time hours. You’ll receive pre-scheduled patient visits, iPad provided, mileage reimbursement, and malpractice insurance. No clinic traffic—work independently and around your locale.

Qualifications

• Active NP license and board certification are required.
• Valid NPI number is required.
• Minimum 3 months of wellness exam experience preferred.
• Comfort with EMR/documentation systems is needed.

Responsibilities

• Conduct in-home wellness assessments.
• Educate members on preventive care benefits.
• Identify gaps in care to reduce ER visits and hospitalizations.
• Complete documentation using EMR tools.
