You control your schedule, choosing full- or part-time hours with clustering in a region. The role offers mileage reimbursement, a provided iPad and supplies, paid training, and pre-scheduled patient appointments through web-based tools.

Qualifications

• Active State NP license and Board Certification.
• Valid NPI and experience with wellness exams preferred.
• Comfort with EMR and documentation systems.

Responsibilities

• Conduct comprehensive in-home wellness assessments.
• Educate members on preventive care benefits.
• Identify gaps in care to reduce avoidable ER visits.
• Complete documentation using provided EMR tools.
