Job summary

Care Connectors in Arkansas is seeking in-home Nurse Practitioners to perform annual wellness visits for Medicare members. These roles are 1099 independent contracts with flexible full-time or part-time hours, offering a guaranteed daily rate and no prescribing.

Appointments are scheduled, mileage reimbursed, and an iPad tablet is provided; travel is within a 30–50 mile radius. Strong independence and patient education focus are essential for success.

Qualifications

• Active State NP license and Board Certification.
• Valid NPI.
• Minimum 3 months of wellness exam experience preferred.
• Comfort with EMR/documentation systems.
• Ability to travel within a 30-50-mile radius.
• Bilingual/Spanish speaking a plus.
• Availability to work 2 days per week.

Responsibilities

• Conduct in-home wellness assessments.
• Educate members on preventive care benefits.
• Identify potential gaps in care to help reduce avoidable ER visits and hospitalizations.
• Complete documentation using provided EMR tools.
• Visits are face-to-face and take place where members live.
