Job summary

Gilder Search Group is seeking a Nurse Practitioner to perform in-home Annual Wellness Visits for Medicare members in Stockton, CA and surrounding areas. This is a flexible, field-based role offering Full-Time, Part-Time, or 1099 contract options with travel within a 30–50 mile radius and no prescribing duties.

You’ll conduct comprehensive wellness assessments, educate members on preventive care, and document using EMR tools.

Qualifications

• Active State NP license and Board Certification.
• Valid NPI.
• Minimum 3 months wellness exam experience preferred.
• Comfort with EMR/documentation systems.
• Ability to travel within a 30–50 mile radius.
• Bilingual/Spanish Speaking.

Responsibilities

• Conduct comprehensive in-home wellness assessments.
• Educate members on preventive care benefits.
• Identify potential gaps in care to help reduce avoidable ER visits and hospitalizations.
• Complete documentation using provided EMR tools.
