Job summary

Gilder Search Group is seeking Nurse Practitioners to perform in-home Annual Wellness Visits for Medicare members. These visits are conversational, consultative, and education-focused—no prescribing, no treatment plans.

Qualifications

• Active State NP license and Board Certification.
• Valid NPI.
• Minimum 3 months of wellness exam experience preferred.
• Comfort with EMR/documentation systems.
• Ability to travel within a 30-50-mile radius.
• Bilingual/Spanish Speaking a plus.
• Give on average 2 days per week.

Responsibilities

• Conduct comprehensive in-home wellness assessments.
• Educate members on preventive care benefits.
• Identify potential gaps in care to help reduce avoidable ER visits and hospitalizations.
• Complete documentation using provided EMR tools.
• Visits are face-to-face and take place where members live.
