Job summary

Care Connectors seeks a Nurse Practitioner to perform in-home Wellness Visits for Medicare patients. This field-based role offers flexibility in scheduling and no office commute, with autonomy in a travel-heavy environment.

The NP will conduct comprehensive assessments in patients' homes, reinforce preventive services, and identify care gaps while documenting findings in EMR systems. Travel across designated markets is required.

Qualifications

• Must be State licensed and Board Certified Nurse Practitioner with NPI.
• Prefer experience with wellness exams and Medicare populations.
• Ability to work independently in home-based settings.

Responsibilities

• Conduct comprehensive in-home Health/Annual Wellness Visits for Medicare patients.
• Reinforce preventive services and identify gaps in care.
• Document findings and coordinate care using EMR.
