Job summary

Gilder Search Group is seeking a Nurse Practitioner to perform in-home Annual Wellness Visits for Medicare members. The role offers flexible scheduling, strong earnings potential, and the independence of 1099 contracting with no prescribing responsibilities.

You will conduct in-home wellness assessments, educate members on preventive care, and document findings using EMR tools. Travel within a 30–50 mile radius is required; bilingual Spanish is a plus.

Qualifications

• Active State NP license is required.
• Board Certification is required.
• Minimum 3 months of wellness exam experience preferred.
• Comfort with EMR/documentation systems.
• Ability to travel within a 30-50 mile radius.
• Bilingual/Spanish speaking a plus.

Responsibilities

• Conduct comprehensive in-home wellness assessments.
• Educate members on preventive care benefits.
• Identify potential gaps in care to help reduce avoidable ER visits and hospitalizations.
• Complete documentation using provided EMR tools.
• Visits are face-to-face and occur in members' homes.
