Job summary

Care Lync is seeking a full-time Support Coordinator to manage individualized care plans, coordinate services, and monitor outcomes for adults with disabilities and autism.

Responsibilities include developing ISP-based plans, brokering services, and ensuring cost-effective, professional care in home and community settings. Travel up to 50% is required to meet participants where they live and access needed supports.

Qualifications

• BS/BA degree in related field is required.
• Experience with Microsoft Office and state-maintained databases.
• Maintain CPR/First Aid certification and state-related Clearances.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan for each participant and update annually or with significant changes.
• Coordinate with providers to ensure informed participant choice of services.
• Ensure alignment of Behavioral and Crisis Plans with the ISP.
• Document incidents of abuse, neglect, or exploitation promptly and report to supervisor.
• Coordinate with other care coordinators and entities to ensure seamless service delivery.
• Research and develop alternative solutions for participant needs.
• Maintain confidentiality of participant information.
• Attend trainings and recertifications as required.
• Communicate participant needs with supervisors and directors.
• Travel up to 50% to participant locations as needed.
