Job summary

BlueCross BlueShield of Tennessee is seeking a Support Coordinator for ECF Choices in Sumner County. You will empower members to achieve better health by coordinating home and community-based services, assessments, and person-centered plans.

Ideal candidates are RNs with relevant certifications or a BS in health/social care, plus 5+ years as a Support Coordinator. Expect travel to members’ homes; mileage is reimbursed, with a day schedule from 8:00am to 5:00pm and no on-call duties.

Qualifications

• RN with current licensure or equivalent developmental disabilities credentials.
• Relevant Bachelor’s degree or higher in health/social care fields is preferred.
• Qualified Developmental Disabilities Professional (QDDP) or QIDP eligibility required or preferred.
• Experience as a Support Coordinator in HCBS waivers is highly desirable.

Responsibilities

• Partner with members and families to identify supports and services.
• Collaborate with clinical and social support teams to meet health and long-term needs.
• Conduct face-to-face visits to assess physical, behavioral, functional and employment status.
• Utilize criteria to authorize appropriate home and community-based services.
• Travel to Sumner County members' homes; mileage reimbursement applied.
